Sliding Scale

Sliding Scale Information

We accept insurance and offer sliding scale to make our care more accessible.

Below you'll find information about our sliding scale rates. In the event that we are not in-network with your insurance or you have limited benefits, we offer a sliding scale for paying out-of-pocket if finances are a barrier to receiving the care you need.

Insurance and Billing Information

Access to care is something we highly value at The Cooked Carrot. We are actively working to expand in-network care with other insurance providers.

We are currently in-network with:

If we do not accept your insurance, we can provide you with a superbill to submit to your insurance company if you have out-of-network benefits. Please note that a superbill does not guarantee reimbursement. Sessions that are not covered by insurance will be the responsibility of the client.

Sliding Scale Structure

The Crooked Carrot Nutrition employs a two-tier sliding scale model inspired by Alexis J. Cunningham's model.

How to choose a tier:

We use a sliding scale pricing model to help ensure nutrition counseling is accessible and affordable. You are encouraged to select the tier that best reflects your current access to financial resources, not just income alone.

When choosing a tier, consider factors such as:

  • Income and financial stability
  • Access to savings or generational wealth
  • Housing security and debt obligations
  • Dependents or caregiving responsibilities
  • Ongoing medical or living expenses

You may also find it helpful to reflect on the "Green Bottle model" as a guide when selecting your tier. See the image below.

The Green Bottle Model

You can use this model to determine where you fall on the sliding scale.
We are currently in-network with:

Those with greater financial flexibility are encouraged to select a higher tier when possible, which helps sustain the practice and expand access for others. If your circumstances change, please let us know so we can adjust your tier to remain responsive and fair.

The Crooked Carrot sits on the traditional lands of the Diné Bikéyah (Navajo), Núu-agha-tʉvʉ-pʉ̱ (Ute), and Pueblos people, both past and present, now called Durango, Colorado. I acknowledge the wounds of the past in hopes of being able to heal for a better tomorrow.
