Pricing for Sessions
I offer nutrition counseling sessions both virtual and in-person in our Durango, Colorado office.
I accept both private pay and insurance.
Initial Session (55 Minutes)
During our 55-minute initial session, we will do a thorough assessment and talk about your relationship with food and your body - past and present. We'll also discuss your goals and next steps to help you get there.
One time

Follow-up Sessions (55 Minutes)
In these 55-minute follow-up sessions, we'll continue diving deeper into your thoughts, feelings, and beliefs around food and your body while finding ways to shift in a way that works for you. How often we check-in is flexible.
Per session

Insurance and Billing Information
Access to care is something we highly value at The Cooked Carrot. We are actively working to expand in-network care with other insurance providers.
We are currently in-network with:
If we do not accept your insurance, we can provide you with a superbill to submit to your insurance company if you have out-of-network benefits. Please note that a superbill does not guarantee reimbursement. Sessions that are not covered by insurance will be the responsibility of the client.
Verifying your Insurance Coverage
We do our best to check your benefits before we start working with you, but sometimes insurance representatives give us incorrect information. We encourage you to also verify your coverage so that you also understand what will be covered by your insurance.
To verify that your insurance plan covers nutrition counseling, call the number on the back of your insurance card and ask the following questions listed below. Be sure to write down answers, along with the reference number so that you can refer back to later if needed.

Questions to ask:
- Is this dietitian/The Crooked Carrot currently an in-network provider for my plan?
- Does my plan cover outpatient nutrition therapy CPT codes 97802 (medical nutrition therapy - assessment) and 97803 (medical nutrition therapy - follow up)?
- Do I have any nutrition counseling visits covered under the preventative care portion of my plan, (ICD-10 code Z71.3)? How many?
- Do I have any nutrition counseling visits covered under the medical necessity portion of my plan? How many?
- Do I have coverage for services provided via telehealth?
- Do I need a physician referral for nutrition counseling?
- Do I have a deductible for nutritional counseling services? If so, how much is it?
- Is there a copayment or co-insurance for each visit? If so, how much?
- How many sessions are covered per year?
- What month does the policy year renew?
- Are there any restrictions and/or limitations to my coverage?

Tips for calling:
- Avoid calling on Monday
- Call early in the morning or later in the day - add to your calendar if that's helpful
- Have all of your information ready (i.e. insurance card)
- Collect the name of the representative, call reference number, and a callback number/the number you are being transferred to before you end the call
Out of Network Benefits
We're not in-network with all insurances, but if you have out-of-network nutrition insurance benefits, we can provide you with a superbill receipt to submit to your insurance company. You may get full or partial reimbursement from them.
This varies depending on the insurance company and the specific plan you have. Many of our clients who do have out-of-network benefits have had success getting partial or full reimbursement, or at least have it put towards their deductible if they have one. The superbill does not guarantee reimbursement. Sessions that are not covered will be the responsibility of the client.
We recommend calling your insurance company to ask about your coverage, so you know if you will receive any reimbursement for our services. I've included the instructions on how to do this below.
When insurance companies agree to cover an out-of-network practitioner under the "in-network" benefits, this is often called Gap coverage. You may also be able to request a single case agreement for MNT depending upon your plan and diagnosis. Should you obtain a single case agreement or GAP coverage, we will require written documentation and information on copay, deductibles and how the insurance company would like to be billed for services.
What if you're not in-network with my insurance company?
We can provide you with superbills so you can then submit them to your insurance company for reimbursement if you have out-of-network benefits. Some insurance companies may require a diagnosis for reimbursement. If this is the case with your insurance company, we'll work together to communicate with your physician and/or therapist to get any relevant diagnoses on file.
How much does nutrition counseling cost?
If The Crooked Carrot Nutrition is not an in-network provider with your insurance plan, you are responsible for payment at the time of service. We accept credit cards, debit cards, FSA cards, HSA cards and bank debits. Sessions are 55 minutes and rates are $145 for a 55-minute initial or follow-up session.
If The Crooked Carrot is an in-network provider with your insurance plan, nutrition counseling may be fully covered by your insurance benefits. Sometimes, you may owe a co-pay or co-insurance for each visit, may have to reach a deductible, or may have a limited number of visits covered per year. Verifying your benefits using the script above will give you some clarity on this.
Sliding scale options are available as well if finances are a barrier.
Will I have to pay for sessions if I'm using my insurance?
A cost-share is the amount you will need to pay as required by your particular insurance plan towards your services. A cost-share can be in the form of a deductible, co-pay or co-insurance.
We will always bill under your insurance policy's plan under your preventative benefits if your plan allows. With that being said, if you have preventative benefits there is often NO cost share for you associated with the visit.
In the event you have a cost-share we will initially bill your insurance company directly. Once we receive the explanation of benefits/payments describing your responsibility as the patient, we will bill the credit card on file for the amount noted under 'patient responsibility'.
For most insurance companies, dietitians are considered a specialist. Therefore, your specialist co-pay is applicable and is payable at the time of service. This information is often apparent on the front of your actual insurance card. However, often because we bill your insurance with preventative counseling the co-pay is often not applicable.
We generally wait for the claim to be processed to determine whether or not you have a copay and then charge the credit card you have on file with us the co-pay amount.
Do I need a referral for nutrition
In most cases, you don't need a referral for nutrition counseling. However, in order for us to submit claims to your insurance plan or provide you with a superbill for potential out-of-network reimbursement, we might need some information from your primary care provider or your therapist (if you've been diagnosed with an eating disorder) in order to fill out the required documentation. If you plan to use your insurance for sessions or want superbills to submit to your insurance if you have OON benefits, please have your medical provider or therapist fill out this form.
